Regional clubs or groups of individuals interested in organizing a PCA-approved Supported Entry at an All-breed show can use this form to help plan, to give ideas on benefits to be asked for from the kennel club holding the show, and to request PCA Board approval.

Send the form to the PCA Corresponding Secretary at least 6 months before the date of the show.
